These changes are reflected in Attachment V, Appendix 1 and Attachment V, Appendix 11. The effective date is January 15, 2013.

Language accepted on 4/29/2013 in Letter Order ER13-1061 to implement changes to two pages of the Formula Rate Template for Lincoln Electric System. These changes are reflected in Attachment H, Addendum 6 (LES). The effective date is January 1, 2013.
